Sliding Fee Scale Program

No Insurance? No problem! HCCH offers a sliding scale for patients who do not have health insurance. The sliding scale copay varies depending on your household size and income. Please plan to bring a 30-day proof of income paystub upon your first appointment to see if you qualify. Renewal applications are required every year in order to keep receiving sliding scale benefits.
