How To Become A Patient

Click "Get your new patient application" below to download and print a copy or pick up a printed copy from any clinic location.

Make sure you bring all your medications, your insurance card if you have one, and any pieces that we're missing from your application. We look forward to welcoming you to High Country Community Health.

Go to the payment options page to make sure we accept your insurance. If uninsured, please review the requirements for our Sliding Scale program. 

COMPLETE YOUR NEW PATIENT APPLICATION

Complete an application for each new patient, even if they are in the same family. 

TURN IN YOUR COMPLETED APPLICATION

Bring your completed application to your preferred clinic. Don't forget to include identification and proof of income.

SCHEDULE YOUR FIRST APPOINTMENT

We will call to find out a time that works for you and your family. Make sure you write down your appointment date and time!
