Carole Ann Church

Family Nurse Practitioner

Carole Ann Church, MSN, APRN, FNP-BC, received her nursing degree from Caldwell Community College. She received her BSN from Appalachian State University and her Master's degree from East Tennessee State University. She is board certified by the ANCC and holds licensure in NC and TN. She began working at High Country Community Health in 2021.


Carole Ann practices Family Medicine and Medication-Assisted treatment for opioid use disorder at both Avery and Primedical locations. Carole Ann also leads the Migrant Farm Worker health clinic at Avery and enjoys learning Spanish to facilitate culturally appropriate care. Carole Ann spent over 15 years in pharmacy and durable medical equipment; has worked at both Ashe Memorial and Watauga Medical hospitals as an RN, and started her FNP career in Nephrology. She is passionate about providing care to patients with behavioral, racial, ethnic, and/or class disparities. When not working, Carole Ann enjoys reading, traveling, and taking care of her plants, dachshunds, and family.
