About the Role

Job Summary and Responsibilities

The medication assistance worker is responsible for providing medication assistance for patients in need across all High Country Community Health offices.
Job responsibilities will include:
Efficient communication and team based care approach with medical and behavioral health staff in receiving in house referrals and requests for medication assistance
Working knowledge and proficiency of acquiring medications through:
Utilizing the North Carolina Office of Rural Health Medication Assistance software
Using local pharmacy assistance programs
Using pharmaceutical company pharmacy assistance programs
Serve as a resource to medical and behavioral health providers for pharmacy assistance programs
Network with other medication assistance programs to utilize best practices
Track and perform data entry for all pharmacy requests, receiving of medications, and distribution or coordination of distribution to patients
Generate periodic reports of activities for grant reporting purposes
Other related patient assistance duties as identified by the care team
Other duties as assigned

Requirements

Qualifications/Skills/Experience

At least two years experience in a medical or human services field.  Proficiency with computers, spreadsheets, data tracking, and internet research. Valid NC driver’s license and good driving record. 

Demonstrated ability to collaborate effectively in a team setting. Ability to communicate clearly and effectively through verbal and written communication.  Ability to be self-guided.  


Travel Requirements

Deliver medications to patient homes and other HCCH offices.
