About the Role
Community Health Workers are frontline public health workers who are trusted
members of and/or have an unusually close understanding of the community served. This trusting
relationship enables the worker to serve as liaison/link/intermediary between health/social services and
the community to facilitate access to services and improve the quality and cultural competence of service
delivery. A community health worker also builds individual and community capacity by increasing health
knowledge and self-sufficiency through a range of activities such as outreach, community education,
informal counseling, social support and advocacy. The primary purpose of these roles is to engage,
support, and build capacity of individuals and communities to improve health outcomes within the Latino
population offering interpretation and translation services, transportation, providing culturally appropriate
health education and information, and helping people get the care they need.
A Community Health Worker provides community outreach services to expand connections to the Hispanic/Latino
population in Surry County for the purposes of building trust in and awareness of COVID-19
prevention practices and services, e.g., testing, treatment, contact tracing, immunizations.
Build and maintain collaborative working relationships with key community partners serving the Hispanic/Latino/a population in Burke county, including but not limited to local community leaders, community based organizations, public health, contract tracers, and primary care providers.
Collaborate with MAHEC to establish and implement referral systems to collaborative partners for providing CHW services to individuals needing to isolate or quarantine due to COVID 19 disease or exposure.
Reach an outreach minimum of 720 Hispanic/Latino/a and families in Burke county, and 250 through individual support services
Collect and submit weekly data to MAHEC and support of regional effort and program evaluation
Use NCCARE360 where appropriate to initial referral for support around social determinant of health needs
Identify and secure resources to support people being served
Requirements
Bilingual in Spanish/English
Able to perform each essential duty satisfactorily
High School Diploma and/or college degree with a focus in Spanish and or social advocacy
